If FIR is not available then MLC (Medico
Legal Certificate) from Hospital / Nursing Home to be sent where emergency treatment was
given to the Patient (This Certificate is must for all Accidental cases).
Doctor Certificate of Disability;
Leave Certificate from the employer;
Photograph of the injured;
Original Medical Bills with supporting
prescriptions of treatment from the treating Doctor.
